血清淀粉酶的测定及其在糖尿病诊治中的应用

【摘要】 目的:运用生化分析仪测定血清淀粉酶,并对血清淀粉酶在糖尿病监测中的应用进行探讨。方法:运用对硝基苯麦芽七苷糖法(PNP-G7) 测定血清淀粉酶。结果:通过对糖尿病人,健康人群,血糖控制正常的糖尿病患者测定血清淀粉酶,糖尿病组的均值明显低于其他两组。结论:上述三组人群中,糖尿病组与后两组有显著差异,但后两组之间无显著差异。因此,我们认为清血淀粉酶测定除了用于胰腺炎,腮腺炎的诊治外,对糖尿病患者的检查和治疗也有一定的临床价值。

Determination of Serum Amylase and Its Application in the
Diagnosis and Treatment of Diabetes

Abstract:Objective We test serum amylase by use of biochemical analyzer and it is explored in diabetes monitoring.Methods We test serum amylase by use of PNP-G7.Results By testing serum amylase of diabetes,healthy population and normal blood sugar in diabetic patients,we found that diabetic group was significantly lower than the average of other two groups. Conclusion In those three groups,the diabetic group is significantly different from the other two groups,but it has no significant difference between the following two groups,therefore,we believe that except the determination of serum amylase is used to treatment of pancreatitis,mumps.It has clinical value for examination and treatment of diabetes too.

淀粉酶主要是由唾液腺和胰腺分泌,是一种重要的消化酶,能分解多糖如淀粉和糖原,同时淀粉酶是一种需钙的金属酶,其合适PH在6.5~7.5之间,卤素和其他阴离子有激活作用。淀粉酶相对分子量约为50 000,易由肾脏排出,患急性胰腺炎和急性腮腺炎时,血中的淀粉酶明显升高。肝炎、肝硬化、肝脓肿、恶病质等疾病时血淀粉酶活性降低。我们在日常工作中发现许多糖尿病人,随着血糖的升高其血清淀粉酶却明显下降,这表明高血糖与血清淀粉酶下降有一定的关联。
1 材料和方法
1.1 样本来源
所有样本来自江苏省扬州市疾病控制中心医学门诊及其下属网点门诊的糖尿病患者,我们所用样本在2006年1月至2007年8月糖尿病的诊断符合WHO糖尿病诊断标准,即患者的两次随机的空腹血糖值>7.0 mmol/L。标本均为患者血清或肝素抗凝血浆,其他抗凝剂能抑制淀粉酶的活性。
